Welcome to the official Time Off Request page for Kennebec Valley Dental Arts. This page is intended for use by current KVDA employees only.

Please use the form linked below to request time off, including vacation, personal days, or medical leave. All requests must be submitted using your @kvdentalarts.com email address in order to be processed.

Important: Requests submitted using personal email addresses will not be reviewed. Once your request has been reviewed, you will receive a response indicating whether it has been approved or denied. All responses will be sent to your KVDA email account.
